Crystal A. Hazelwood, 62, of Shelbyville, passed away Thursday, June 3, 2021 at

MHP Medical Center.

Born July 8, 1958 in Shelbyville, she was the daughter of Alex Spurlock and Daisy

(Barger) Spurlock.  She married Allen Hazelwood on May 19, 1979 and he preceded

her in June of 2000.

Survivors include 3 children- Paul Hazelwood (fiance Liz) of Brown County, Gary Hazelwood

of Shelbyville, Christopher Hazelwood (wife Amanda) of Indianapolis; 2 brothers- James Spurlock

(wife Sue) of Columbus, Jim Spurlock (wife Karen) of Shelbyville; nieces Stephanie Davis, Ashley

Spurlock; nephew Jay Fleener; 10 grandchildren- Zoe, Paige, Meghan, Marcus, Morgan, Emma,

Lance, Seth, Dakota, Hailey; and 5 great grandchildren- Rylie, Elana, Malakai, Harper, and LuLu.

She was preceded in death by her parents, her spouse, a sister Emma Fleener, and 2 grandsons-

Ethan Hazelwood and Jacob Hazelwood.

Mrs. Hazelwood had lived in this area most of her lifetime, and attended Shelbyville High School.

She was a home maker.

A Celebration of Life will be held at a later date.
